Hi,

for form_test.m, I

+ added a lacking import of module maybe,
+ changed the url to the Apache httpd default CGI dir 
http://localhost/cgi-bin/
+ moved the display of the input form out of the conditional (so it can 
be used at first call)

It is sufficient now to move the compiled binary `form_test' into the 
Apache httpd default CGI dir.
