[m-rev.] diff: make foreign_procs contiguous in io.m and exception.m

The only part of the diff that may be of interest is the deletion of --no-halt-at-warn
for io.m. The rest just moves code around. The only other change I did was to
replace comments that preceded a group of foreign_procs with equivalent comments
on each foreign_proc (since the reordering scatters the members of the group).
