[m-rev.] Fixes to pprint and change for --dump-mlds

Having looked at the test results from running bootcheck on the
proposed change, I have identified several problems with pprint.
(Sorry, Mark - hope you haven't wasted too much time on the review.)

Once I've fixed these problems I'll aim for three separate
changes to be reviewed separately:

1. Fixes to pprint, including some minor aesthetic code changes,
   and test cases.
2. A trivial change to make --dump-mlds use the pretty printer.
3. Extensions to pprint to format pre/post/infix operators correctly.
