BSD (updated, from http://www.opensource.org/licenses/bsd-license.php) looks 
like this:

     Copyright (c) <YEAR>, <OWNER>
     All rights reserved.

     Redistribution and use in source and binary forms, with or without
     modification, are permitted provided that the following conditions are met:

         * Redistributions of source code must retain the above copyright
     notice, this list of conditions and the following disclaimer.

Clauses 1 and 2 meet the original "all I want is that my credit is
recognized and kept in comments" desire, as far as I can tell.
