Definitely in 2002, since then I'm not sure - the last substantial
changes committed regarding Visual C were committed in 2004, so
presmuably then.

ntwin32.mak is part of (or at least provided with) the windows SDK.

Possibly not, I only mentioned it on the possiblity that the build process
for the Boehm collector had changed between versions.
