This does the first step: eliminating the hlc_nest and hl_nest grades.
For post-commit review by anyone. (The diff is with -b.)

After the diff, there are no occurrences in my workspace of

MR_USE_GCC_NESTED_FUNCTIONS
gcc-nested-functions
gcc_nested_functions
gcc-local-gotos
gcc_local_gotos
MR_NestedCont

There are still references to "nested functions", but that is ok,
since we do still generate them internally (before flattening them).
