Attached are

- a diff for an almost-complete rewrite of the reference manual
  section on higher order insts and modes, and

- the diff for improving the error message for passing higher order values
  without higher order insts, along the lines in the previously attached proposal.

For post-commit review by anyone.

In particular, the diff to the manual has several ZZZs (all commented out)
that I would like comments and opinions on, and which I hope to resolve
over the next week or so.
