It's currently available in the CVS repository under books/tutorial.

It's in rough shape and the latter few chapters (mainly more esoteric
stuff) are just shells.  However, any feedback would be very gratefully
received - if there is material that isn't clear or which looks like it
will be absent, please let me know.  Ideas for examples and so forth
would also be helpful.
