[m-rev.] for post-commit review: carve five new modules out of goal_util.m

For review by anyone. The diff is big, but the only part worth reviewing
is the change to notes/compiler_design.html, including the names of the
new modules. The rest is just code being moved around, or updates to
include_modules.
