[m-rev.] for review: io.primitives_{read,write}.m

The diff just moves code and adds some module qualifications,
so the only thing to review is the module naming scheme.

It has been bootchecked in asm_fast and java, and a C# bootcheck
is building stage 3 right now.

In the absence of a review by then, I intend to commit this
in a couple of hours.
