[m-rev.] for review: rename the .ll_debug grade component for hlc grades to .c_debug

I am particularly seeking opinions on the XXX in the log file:
the treatment of the --c-debug option to mgnuc.

Note that the diffs to the files in the runtime other than mercury_conf_param.h
were done by a global search and replace, and don't need to be looked at.
