[m-rev.] diff/for review: replace __ with . as module qualifier in the library

library/*.m:
	Replace __ with . as the module qualifier everywhere.

tests/hard_coded/test_injection.exp:
	Replace __ with . as the module qualifier in expected exceptions.

The diff is huge and not very interesting; it is in ~zs/tmp/DIFF.lib_dot.
Unless someone is a masochist, I am expected review comments only about
the idea.
