[m-dev.] Suggest s/mutvar/mvar/ in extras/concurrency

The name `mutvar' is exported from both extras/concurrency/mutvar.m
and library/store.m.

Although the types involved have different arities, I think the
clash is unfortunate.  How hard would it be to change the
extras module and type name to just `mvar' in line with its
Haskell origins?  If the change t'were done, when t'is done,
t'were best done quickly :)
