bugs in intermod.m

Hi Simon,

There is some code in intermod.m around line 500 --
the predicate fix_function_or_higher_order_in_unify_rhs --
which assumes that only higher-order terms can be
module qualified.  This assumption is no longer true,
now that you've implemented module qualification of functors.

Could you please fix this?
