Changes to mercury/runtime

People,

Tomorrow (Wednesday) I will be changing all code in mercury/runtime 
to conform to the new `C coding standard for the Mercury project'.

As I mentioned before, the coding standard is in:

If you have any suggestions on the coding standard or don't want
me to hit all the code immediately, speak now or forever hold 
your peace. 8^)
